Briefly describe the organization and its purposes, geographical area and people served.

Describe the project or program, dates, whom it would benefit and how. If only partial funding is received, will you proceed? From which other entities have funds been requested?

• Organization brochures or literature
• Budget summary showing expenses and other income sources
• Most recent annual report, if available
• Copy of Federal IRS tax-exempt determination letter (Note: Requests will NOT be considered without submittal of this letter)
